The sanding disk flew off the base as soon as I started sanding. And I can't find any directions to be sure I put it on correctly.

For solid base sanders! Then say so!
Packaging should say that they are specifically for solid or rubber base sanders. Packaging says for Orbital & DA sanders. But not all orbital sanders have a solid base, mine has hook & loop (Velcro like). If you would say so on the packaging then you'd save a lot of people the headache, time and money.

wont stick to pad
tried disc sanding pad and triangler pad and neither stuck to pad. terrible product.

These do not stick! The minute you turn on sander they go flying right off. Waste of money!
